ANNEX 1 - MANDATORY CONDITIONS
1 Supply of Alcohol
1. No supply of alcohol may be made under the premises licence -
(a) at a time when there is no designated premises supervisor in respect of the premises licence, or
(b) at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
2. Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
2. Door Supervision
1. Any person(s) required to be on the premises to carryout a security activity must be licensed by the Security Industry Authority.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
1. The cctv system installed shall be well maintained and shall be operative at all times the premises are open of the public. Visual images shall be retained for a minimum of thirty one days and shall be handed to the police on request.
2. Delivery of goods only to be made during daylight hours.
3. Adoption and implementation of Challenge 21 scheme or similar initiatives whereby only recognised proof of age cards accepted e.g. passport, Portman group, UK photo id driving licence.
